Rooted Health Hormones & MedSpa, LLC, our clinicians, employees, and Business Associates acting on our behalf.
Uses and disclosures we may make without your authorization
- Treatment — to provide, coordinate, or manage your care (e.g., labs, pharmacies, consulting clinicians).
- Payment — to bill and collect for services.
- Health-care operations — quality improvement, audits, training, accreditation.
- Required by law — public health reporting, abuse/neglect reporting under Ark. Code § 12-18-402, court orders, law-enforcement requests, FDA adverse-event reports.
Uses requiring your written authorization
- Marketing communications (other than face-to-face).
- Sale of PHI (we do not sell PHI).
- Most uses of psychotherapy notes.
- Other uses not described in this notice.
You may revoke an authorization in writing at any time, except for actions already taken in reliance on it.
Your rights
- Access — inspect and obtain an electronic or paper copy of your records.
- Amend — request corrections to your record.
- Accounting of disclosures — request a list of certain disclosures we made.
- Restrict — request limits on uses or disclosures (we will agree if you pay out-of-pocket in full for a service and request the disclosure not be sent to a health plan).
- Confidential communications — request alternate means or locations.
- Paper copy of this notice — at any time on request.
- Breach notification — be notified of any breach of your unsecured PHI.
Our duties
We are required by law to maintain the privacy and security of your PHI, provide this notice, and abide by its terms currently in effect. We may change this notice; the revised notice will apply to all PHI we maintain.

You may file a complaint with our Privacy Officer at privacy@rootedhealthmember.com or with the U.S. Department of Health & Human Services, Office for Civil Rights, 200 Independence Ave SW, Washington, DC 20201, 1-877-696-6775, hhs.gov/ocr. We will not retaliate for filing a complaint.
Effective date
This notice is effective May 13, 2026.
