This document describes the nature, benefits, and risks of receiving care from Rooted Health by telehealth. Telehealth involves the use of secure electronic communications to provide clinical services without an in-person visit.
Eligibility
Telehealth is available only to patients physically located in Arkansas at the time of the visit, consistent with the Arkansas Telemedicine Act (Ark. Code § 17-80-401 et seq.) and Arkansas State Medical Board / Arkansas State Board of Nursing rules.

Risks & limitations
- Some conditions cannot be fully evaluated without an in-person exam, labs, or imaging.
- Technology failures (connectivity, audio/video) may interrupt or delay care.
- In rare cases, electronic transmission may be intercepted despite encryption safeguards.
- Controlled substances generally require an in-person evaluation under federal Ryan Haight Act rules; we will refer you appropriately.
What to do in an emergency
If you are experiencing a medical emergency, do not use telehealth. Call 911 or go to the nearest emergency department. For mental-health crises, call or text 988 (Suicide & Crisis Lifeline).
Privacy
Telehealth sessions and messages are subject to our HIPAA Notice of Privacy Practices.

By scheduling a telehealth visit, you acknowledge that you have read this consent, had the opportunity to ask questions, and agree to receive care by telehealth. You may withdraw consent at any time and request an in-person visit.
